TOUR OF DUTY: This is a full-time position.
PRIMARY DUTY LOCATION: This position is based out of the Loch Raven CBOC, with clinic days scheduled at the Fort Howard CBOC, but travel throughout the VAMHCS (mainly the Baltimore VAMC) may be required.
INCENTIVES: Relocation Expenses, and/or Recruitment/relocation incentives are not authorized for this position.
OVERVIEW:
A full-time position exists for a Mental Health Nurse Practitioner (NP) for the VA Maryland Health Care Center (VAMHCS) to work in the Community Based Outpatient Clinics (CBOCs).  This position is based out of the Loch Raven CBOC, with clinic days scheduled at the Fort Howard CBOC, but travel throughout the VAMHCS (mainly the Baltimore VAMC) may be required.   The selected individual’s primary assignment will be to assure assessment and medication management of patients presenting to the CBOC for mental health treatment. Additionally the selected NP will provide telehealth services to veterans on the Eastern Shore.  The incumbent must perform at an advanced level of nursing practice, responsible for the integrity, uniformity, continuity and quality of clinical nursing practice. The incumbent reports directly to the Associate Chief Nurse, Mental Health for nursing practice and to the Sub-Product Line Manager through the specific program coordinators for programmatic needs and team functioning.

OVERVIEW:
A position exists for a full-time Case Manager in Infectious Disease, for the outpatient care of HIV infected veterans, as well as those on long term outpatient IV antibiotics, in the Medical Care Clinical Center, Baltimore Medical Center, VA Maryland Health Care System.  The Infectious Disease Section provides primary care to veterans with HIV. The Case Manager (CM) is a professional nurse who manages an outpatient caseload of veterans through coordination of care with physicians, nursing staff, and other health professional within the hospital systems. The incumbent performs at an advanced level of nursing practice characterized by both increased complexity of clinical decision-making and enhanced skills in managing patient populations in a specialized primary care setting. He/she utilizes the nursing process within his/her scope of practice, in identifying health issues/needs of patient with HIV or those veterans on long term IV antibiotic treatment and communicates health concerns to the  Infectious Disease/Primary Care Provider in a timely manner.

The Case Manager is directly accountable to the Nurse Manager, Medical Specialty Clinics, and ID Lead Nurse Clinic Specialist and in a matrix to the Chief of Infectious Disease. The position is an eight hour tour of duty, day shift.

    Position Summary:
